From the vid, I see bullet failure. By the time the bullet decided to tumble, it would have passed thru most self defense targets. Hard to beat the Lehigh Defense stuff though. Take a look at these wound channels in gel:

As I recall, Underwood loads the Lehigh bullets. You can load your own getting them from Midway USA.

2" to 2.5" neck certainly isnt perfect but pretty far from failure IMO.
